All information submitted is kept confidential. This assessment form serves as a basis for an initial understanding of the dynamic elements which contribute to an individual's current level of psychological and emotional functioning. However, once a formal application is made to a treatment facility a complete assessment should be undertaken that includes information elicited through selected questioning of the individual. It is the sole responsibility of the applicant to research each facility and/or counselor and determine the level of care that will be provided. Restoring Family can not be held liable for the behavior or quality of care that these facilities and/or counselors may or may not provide.
